For valuable consideration. I for myself, my successors, heirs, assigns, executors, spouse, administrators and next of kin covenant not to sue,
release, waive and discharge B-Ballers Hoops School and Triple Threat Academy and their partners and affiliates from all liability, loss, damage,
claims, demands, causes of action, costs and expenses including attorney’s fees against B-Ballers Hoops School and Triple Threat Academy and
their partners and affiliates arising from myself or my child’s participation in sessions and transportation associated with sessions. I assume all risks
or personal injury and death including medical/hospital bills and damage to personal property arising from myself or my child’s participation in
sessions and transportation connected with sessions except if injury/death is caused by B-Ballers Hoops School and Triple Threat Academy and
their partners and affiliates gross negligence. I understand B-Ballers Hoops School and Triple Threat Academy and their partners and affiliates
provides no insurance for participants.
The signer grants permission to B-Ballers Hoops School and their partners and affiliates, to utilize the Minor/Parent/Guardian image, likeness, actions, and statements in any live or recorded audio, video, or photography, exhibition, plublication or reproduction, without further authorization or compensation. I, the undersigned, do hereby consent to participation in all activities. Therefore, in consideration for the services to be performed by B-Ballers Hoops School and their partners and affiliates, I do further release their agents and employees and another associated with the academy from any and all claims and liabilities to minor/Parent/Guardian for any damages or injuries, which may be sustained in connection with B-Ballers Hoops School and their partners and affiliates.
The contagious nature of COVID-19 and that the CDC and many other public health authorities still recommend practicing
social distancing.
B-Ballers Hoops School and their partners and affiliates has put in place preventative measures to reduce the spread of COVID-19.
B-Ballers Hoops School and their partners and affiliates can not guarantee that I will not become infected with the COVID-19.
The risk of becoming exposed to and/or infected by COVID-19 may result from the actions, omissions, or negligence of
myself and others, including, but not limited to, coaches and their staff.
I voluntarily seek services provided by B-Ballers Hoops School and their partners and affiliates and acknowledge that I
am increasing my risk to exposure to COVID-19. I acknowledge that I must comply with all set procedures to reduce the
spread participating in sessions and/or tournaments.
•I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing,
fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell
•I have not traveled internationally within the last 14 days
•I have not traveled to a highly impacted area within the United States of America in the last 14 days
•I do not believe I have been exposed to someone with a suspected and/or confirmed case of COVID-19
•I have not been diagnosed with COVID-19 and not yet cleared as non contagious by state or local public
health authorities
•I am following all CDC recommended guidelines as much as possible and limiting my exposure to COVID-19
I hereby release and agree to hold B-Ballers Hoops School and their partners and affiliates harmless from, and waive on behalf of myself, my heirs,
and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss
to myself and/or property that may be caused by any act, or failure to act of B-Ballers Hoops School, or that may otherwise arise in any way
in connection with any services received from B-Ballers Hoops School and their partners and affiliates. I understand that this release discharges
B-Ballers Hoops School and their partners and affiliates from any liability or claim that I, my heirs, or any personal representatives may have
against B-Ballers Hoops School with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or
in connection to, any services received from B-Ballers Hoops School and their partners and affiliates. This liability waiver and release extends to
B-Ballers Hoops School together with all owners, partners, and employees.
